two questions

I have been fortunate to have had great success breastfeeding my adopted children. We are thinking about adopting an infant in about a year.
Question 1. How long can I keep up a decent milk supply with pumping alone?
Due to the large size of my nipples I have always needed to use the extra large glass horns with my Pump in Style. As a result of the constant nursing the nipples seem to have increased in size and now even the extra large are too small. I am not exaggerating, my physician has confirmed that I fall outside "normal" range. No problems just big. I've not needed to pump much for the past two years. I've started again and now I find the horns too small, my nipples clog up the tube and pumping hurts.
Question 2. Can I get the horns custom made or is there a better style pump I might try? Thanks
